Abstract
Testudinidae A captive female Greek tortoise, Testudo graeca, at least 35 years of age, weighing 1.15 kg and measuring 20 cm of carapace length was evaluated for anorexia and lethargy of two months duration in the summer of 1996. Weight was considered normal in comparison with control data base from our center, (Testudo graeca of 20 cm weight from 1.00 to 1. 5 kg, n=38). Physical exam revealed edematous posterior extremities and a large number of whitish nodular lesions located throughout the oral cavity. A blood sample from the dorsal coccygeal vein and an oral biopsy were obtained under manual restraint. Cytology of the palatal biopsy revealed uric acid. For plasma biochemistries one ml of heparinized blood was transferred to a serum separator tube and centrifuged at 3000 RPM for five minutes. Plasma biochemical evaluations were made at a commercial laboratory using a chemical analyzer. Blood values for aspartate aminotransferase (AS T. formerly SOOT), uric acid (UA), creatinine were compared to normals from ten healthy Greek tortoises. This revealed a UA level of 1487.5 umol/L (25 mg/dl) (control values from the ten healthy tortoises were between 85.1 to 147.6 umol/L (1.43 to 2.48 mg/dl), AST 360 IU/l (control tortoises: 15 to 226 IU/l and creatinine 22.1 umol/l (0.5 mg/dl) (control tortoises: 4.42 to 57.46 umol/l, 0.1 to 1.3 mg/d). Gout was diagnosed from hyperuricemia and gouty tophi. Empirical treatment was initiated consisting of 20 mg/kg allopurinol (Zyloric, 100 mg) PO q 24 hours at 30 degreesC (86 degreesF) and 20 ml/kg Ringer's solution intracoelomically q 24 hr. On day 21 of treatment, 250 mg probenecid (Blenox, 1 gm/60ml) PO BID was given for 45 days in addition to the allopurinol. Blood was analyzed for UA, creatinine, and AST at 15, 36, 51, 64, and 75 days after initiation of allopurinol and fluids. The results for UA and AST are presented in tables 1 and 2.
